PRIVATE PAYMENT – Payment may be made by FSA/HSA, debit or credit card.  

AETNA – In Network Provider via Headway.  To use Aetna insurance, please establish a free Headway.com account.  Headway will only be used to process your claims – not for scheduling appointments. 

MEDICARE PART B – In Network Provider.  Please include your Medicare ID#, your address, and your date of birth when scheduling your initial consultation.

ALL OTHER PLANS – Out of Network Provider.  Payment is made at the time of our sessions.  Superbills are provided.  Please see below for more information about Out of Network services.

PAYMENT OPTIONS – FSA/HSA, Debit Card or Credit Card.

Billing and Insurance
I use Headway to process all Aetna claims and fees.

I use JaneApp and Stripe to process all other professional fees.   “ML.LCSW” will appear on your statement. 

If you are filing for Out of Network reimbursement with your insurer, I supply a Superbill (statement) any time you request it.  Please see below.

Please contact your insurance company and ask:

1. Does my policy include Mental Health / Behavioral Health Out of Network benefits?
2. What is the “Usual & Customary” fee for a 90837 CPT Code (60 minutes outpatient therapy) in my Zip Code?
3. What percentage of the “Usual & Customary” fee will I be reimbursed?
4. What is the exact dollar amount I will be reimbursed per therapy session?
5. How soon can I expect to be reimbursed for each session?

Click on any Superbill link above, complete the information and submit as requested.  Or send me an email with “Superbill Request” in the subject line to [email protected].  You must list the dates of service you would like included on your Superbill along with your residential address and date of birth each time you request it.  

2-Day | 48 Hour Cancellation & Rescheduling Policy
Please note I require a minimum of 48 hours notice for cancellations and rescheduling.  Because I hold your appointment time exclusively for you, and I am unable to offer it to others, appointments cancelled within 48 hours are billed at your full fee.  Thank you for your understanding.
