Payment Methods Accepted
Federal Medicare Part B (effective 4/15/2024.)  I file claims directly.

Out-of-Network for all other plans.  I provide you a Superbill to submit to your insurer.

Private payment using FSA/HSA, debit and credit cards (all).  Using FSA/HSA pre-tax income saves 30%+.

Forms & Paperwork              120./60 minutes

10 Therapy Sessions           2,000./60 minutes each, pre-paid (17% discount)

Billing and Insurance
I use JaneApp and Stripe to process all professional fees.   “ML.LCSW” will appear on your statement. 

If you are using your Medicare Part B, there is no action required on your part.
If you are using your Out of Network benefits, I will provide you a Superbill on request for submission.

How Much Will Your Insurer Reimburse You for Out-of-Network therapy?
If you have Out-of-Network benefits, contact your insurance company and ask:

1. Does your policy include Mental Health / Behavioral Health Out-of-Network benefits?
2. What do they consider the “Usual & Customary” fee for a “90837” (60 minute outpatient therapy) in your Zip Code?
3. What percentage of this “Usual & Customary” fee will you be reimbursed?
4. What is the exact dollar amount you will be reimbursed per session?
5. How soon can you expect to be reimbursed for each session?

To Request a Superbill
Click on any Superbill link above, complete the information and submit as requested.  Or send me an email with “Superbill Request” in the subject line to Melissa@MelissaLesterLCSW.com.  You must list the dates of service you would like included on your Superbill.  You will receive your Superbill by email and may then submit it to your insurer.

2-Day | 48 Hour Cancellation & Rescheduling Policy
Please note I require a minimum 48-hour notice of cancellation and rescheduling.  Because I hold your appointment time exclusively for you, I am unable to offer it to others.  Appointments cancelled within 48-hours are billed at full fee.  Thank you for your understanding.
